SummaryIn accordance with the National Scope and Standards of
Diabetes Educators, as well as the National Standards for Diabetes
Education and Support, the Diabetes Nurse Educator assists with all
diabetes center programs, counsels individuals, teaches diabetes
classes, and completes office duties and other Lifestyle Center
functions as assigned. Interacts with an interdisciplinary team of
health professionals. Promotes positive behavior change with client
population using motivational interviewing techniques, goal setting
and effective client communication and support. Evaluates and
assists clients with barriers to change.Essential Duties and
Responsibilities These duties and responsibilities described below
represent the general tasks performed on a daily basis; other tasks
may be assigned.
- Provide one-on-one and group outpatient diabetes education and
support.
- Instruct clients on blood glucose monitoring using current
meters/devices.
- Instruct the patient, and their family upon request, on insulin
injection to include drawing and injecting insulin, site selection,
site rotation, storage, action of insulin, syringe/needle disposal,
and treatment of low and/or high blood glucose readings.
- Provide diabetes self-management education and support in all 9
content areas as defined by the National Standards of Diabetes
Self-Management Education and Support.
- Maintain knowledge of billing requirements for major third
party insurance companies and explain these requirements to
patients as necessary.
- Direct patients with payment/billing questions to appropriate
CRH resource.
- Provide information to program participant and the general
public regarding diabetes related services and diabetes center
programs offered.
- Assist in the implementation of the assessment plan, and the
implementation and evaluation stages of the nursing process.
- Assist in developing and revising programs for individual and
group interventions.
- Participate in the Diabetes Program Continuous Quality
Improvement Plan.
- Participate on the Diabetes Advisory Committee.
- Assist with Diabetes Support groups, scheduling speakers and
classrooms.
- Assist with scheduling diabetes group classes, and coordinating
speakers.
- Enter patient billing charges daily.
- Support the hospital's mission by supporting community
educational events.
- Promote the diabetes program through marketing to physicians
and medical office groups.
- Promote fiscal integrity of the Diabetes Program by adjusting
schedules and class offerings as needed.
- Assist with program certification (ADA Education Recognition or
AADE Accreditation).Education and ExperienceMinimum Required
Education: A.D. in NursingPreferred Education: B.S. in
NursingExperience: 2+ years teaching or diabetes experience is
preferred.Certificates, Licenses, RegistrationsApplicants must be
licensed as a Registered Nurse and able to work in the Commonwealth
of Virginia. In addition, applicants must be CDE certified or
certification eligible; if certification eligible, applicant must
be certified within 24 months of employment.
