City/StateVirginia Beach, VAWork ShiftFirst (Days)Overview:Sentara Home Care Services Chesapeake is hiring a full time LPN.Schedule8am-5pm, M-FOn call rotation 2-3 times per month (must have availability to work 5pm-Midnight)OverviewThe Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.Education

• Trade School Graduate (Required)Certification/Licensure
  • LPN License (Required) - Active in Virginia
  • BLS (Required within 90 days)Experience
    • Serve as patient advocate in the prevention of illness or disease
    • Provide care and treatment for patients and clinical support and teaching for patient and family members
    • Customer service, testing, medication administration and the implementation of nursing interventions and care plans
