positively to the community! Responsibilities About the Role During
visits that may last up to one hour, our providers assess and
observe a member’s health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) aims to enhance care
quality and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, which include reviewing
medical history, diagnosing and treating conditions, providing
health education, making physician referrals, and ensuring thorough
documentation in line with our protocols. Administering and
ordering necessary point-of-care and lab tests. Collaborating with
physicians, case managers, social workers, family members, and
other relevant medical personnel. Working alongside Primary Care
Physicians (PCPs) to educate patients and follow up as needed.
Delivering services at the member’s residence, whether at home or
in Skilled Nursing Facilities. Benefits of Working with Us
Consistency – Our assessments are conducted from Sunday to
Saturday, 8 am to 7 pm, but full-time providers typically work
Monday to Friday, 8:00 am to 5:30 pm. Competitive Pay – Enjoy an
annual base salary ranging from $95,500 to $158,400 , determined by
market analysis and experience. Incentive Program – Participate in
monthly (uncapped) and year-end incentive opportunities.

Education and Support – We prioritize your growth! Along with
extensive onboarding training, you will be paired with a preceptor,
receive personalized feedback, and gain free access to online
continuing education. Additional Licensure – If business needs
arise, we will assist you in obtaining additional state licensure
and credentialing in neighboring states or others you may wish to
visit while conducting health assessments. Qualifications Skills &
Experience That Shine A Master’s Degree or equivalent experience
along with successful completion of NP or PA licensure. Current
NP/PA licensure in the state of practice, including prescription
authority or the ability to obtain it as required. Board
certification from NCCPA, AANP, AACN, or ANCC in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialties. Current BLS, ACLS, or CPR
certification. Preferred experience of at least one year as an NP
or PA; new graduates are encouraged to apply! Willingness to travel
up to 15% of the time, including overnight stays, as necessary. A
valid state driver’s license and proof of adequate automobile
insurance for your state of residence. Proficient computer skills
and familiarity with health/medical record software. Exceptional
