Job Description Job Description Description: Occupational Summary Provide physical therapy services in alignment with the patient’s plan of care. Conduct comprehensive initial and ongoing assessments, evaluate patient responses to therapy interventions, and accurately document all evaluations and treatments provided. Reports to the Home Health Manager. Duties and Responsibilities Deliver physical therapy services in accordance with professional standards and agency policies Complete timely and accurate documentation of all therapy interventions and patient responses Perform comprehensive assessments and reassessments of patients and families as therapy progresses Collaborate with physicians and the interdisciplinary team to develop and adjust individualized plans of care Implement skilled physical therapy interventions with a sound understanding of available treatment options Provide therapeutic treatments and assess the need for assistive devices or medical equipment Offer guidance and consultation to patients, families, and other agency personnel as appropriate Evaluate patient progress, complete OASIS assessments accurately, and adjust care plans accordingly Facilitate discharge planning, including final assessments and care recommendations Supervise Physical Therapy Assistants at least every 30 days and Home Health Aides every 14 days, when applicable Participate in required staff meetings, in-service training, and continuing education programs Other duties as assigned. Requirements: Current, valid state license to practice as a Physical Therapist Graduation from a physical therapy education program approved by: The Commission on Accreditation in Physical Therapy Education (CAPTE), or A successor of CAPTE, or An internationally recognized equivalent program, evaluated by a credentialing agency approved by the APTA or listed in 8 CFR 212.15(e) Successful completion of a state-approved licensing examination Minimum of two years of relevant clinical experience preferred Current BLS (Basic Life Support) certification Proficiency in reading, writing, and speaking English Excellent verbal and written communication skills Ability to meet deadlines and work independently with minimal supervision Physically able to climb two flights of stairs and stand/walk for a minimum of four hours Capable of lifting, pushing, or pulling 20–50 lbs on a regular basis Able to reach, stoop, bend, and kneel safely using proper body mechanics in patient home settings Proficient computer skills with the ability to learn and use agency software Valid state driver’s license Satisfactory Motor Vehicle Record (MVR) as required by agency and insurance provider Proof of current auto insurance Availability to work evenings and weekends as needed
