The PT will develop and implement individualized treatment plans to improve patient mobility, function, and quality of life. Key Responsibilities Evaluate and assess patient physical condition and mobility Develop and implement individualized treatment plans Provide therapeutic exercises, manual therapy, and modalities Monitor patient progress and adjust interventions as needed Document patient care and progress notes in compliance with facility policies Collaborate with other healthcare professionals to optimize patient outcomes Educate patients and caregivers on exercises, mobility, and safety Follow infection control and safety protocols
